COVID-19 Status Reporting
COVID-19 Status
Are you symptomatic?
Have you been exposed to someone who is COVID-19 positive?
Have you tested positive?
If you answered yes to any of these questions, you must do the following.
Students
- Quarantine until further guidance is received from Contact Tracing/University Health Service (UHS) personnel and do not attend any in-person activities.
- Notify the following.
- Office of Student Affairs (OSA): Drs. TenHoeve (tenhoeve@uic.edu) or Agard (agard@uic.edu)
- Director of Experiential Education: Dr. Allen (Chicago students: sallen7@uic.edu) or Dr. Schriever (Rockford students: aes@uic.edu); Refer to COVID-19 Guidance Document on eValue
- Preceptor (if you are on an IPPE or APPE)
- Report your COVID status.
- When completing the status reporting survey, please do the following.
- Answer “YES” to the following question, “Are you a student on a clinical rotation currently, in the past two weeks, or in the next two weeks?” regardless of whether your IPPE or APPE falls within the timeframe in the question.
- List Dr. TenHoeve as your supervisor, if asked.
- Click on the Reporting Tool link to complete the status reporting survey.
- When completing the status reporting survey, please do the following.
- Inform OSA, Dr. Allen/Dr. Schriever, and your preceptor (if on an APPE/IPPE) about guidance you receive from Contact Tracing/UHS.
- Notify your course coordinators of need to quarantine if directed to do so by UHS/Contact Tracing. You may participate in remote activities if you’re well enough.
- Return to in-person activities once you’ve received clearance from UHS/Contact Tracing and you have informed OSA (and Dr. Allen/Dr. Schriever and your preceptor if on an APPE/IPPE). Share your clearance with your course coordinators.
Employees
- Quarantine until further guidance is received from UHS or Contact Tracing personnel and do not attend any in-person activities.
- Report your COVID status using the Reporting Tool.
- Notify your supervisor of the need to quarantine. You may participate in remote activities if you’re well enough.
- Return to in-person activities once you’ve received clearance from the UHS or Contact Tracing personnel.
