JOB SUMMARY

Participates in Medication Utilization Evaluations (MUE), education, patient monitoring, Quality Improvement (QI) programs and drug information activities of the pharmacy. Interprets physician orders, evaluates patient profiles, makes appropriate interventions as needed, verifies computerized physician medication orders, admixes or dispenses medication (including chemo) for patients as prescribed by the physician, and practices and promotes the concept of patient-focused care.

RESPONSIBILITIES
Essential Functions
 Demonstrates a commitment to the mission of Swedish Hospital and demonstrates a service orientation and adheres to all responsibilities and standards of the Hospital.

  1. Develops and maintains a clinical practice by providing patient care service and cooperating with medical and nursing personnel to optimize the pharmacotherapeutic aspects of patient care. Provides optimal selection of pharmacologic agents, designing a regimen to maximize therapeutics benefit and minimize toxicity. Provides daily follow-up on the clinical effects of the regimen with adjustments as necessary to achieve desired outcomes.
  2. Verifies computerized medication orders accurately, makes appropriate changes or additions to the patient’s therapy or treatment plans when necessary, verifies compounded medications via dispense prep, and reviews mediation use for compliance with 340B requirements.
  3. Provides drug and clinical information to other healthcare professionals in response to specific questions on newly evolving issues related to the care of infusion center patients. Provides professional education regarding new drugs, biologics, or technologies for specialized drug delivery. Provides a significant level of education to professionals, trainees, and patients. Responsible for monitoring and assessing performance of the learner and providing feedback using appropriate learning objectives that are aligned with the desired learning outcomes.
  4. Supervises and trains pharmacy technicians.
  5. Orders medications and supplies and maintains inventory of the infusion center pharmacy.
  6. Ensures compliance with patient, quality, and USP standards and maintains quality assurance documentation.
  7. Designs, supports, and implements policies to provide effective and efficient use of resources ensuring that budget, quality, and value targets are met in collaboration with the Director of Pharmacy.
  8. Develop, review, assist, and maintain, in collaboration with members of the Department of Hematology/Oncology, various protocols and regimens, participating in testing of therapy and treatment plans build within the electronic health record.
  9. Participates in infusion related committees and department staff huddles.

PATIENT CARE/AGE SPECIFIC RESPONSIBILITIES AND QUALIFICATIONS

  • Makes appropriate clinical interventions according to patient age, height, weight, sex and condition for appropriate indications and dosing.
  • Monitors drug interactions, drug allergies, ADRs, and contraindications.
